Anesthesia in the Geriatrics

Abstract
|
|
|
The practice of geriatrics was essentially defined by the legislation creating the Medicare program as the case of those individuals age 65 or older. Actually, however, there is no definable lower limit in terms of chronologic age, rather there is a continuous physiologic aging process. Some patients "age" faster than others. Aging starts at birth, and physiologic function begins to deteriorate measurably after 30-40 years. Anesthetic problems in a geriatric patient are, therefore, not unique to the elderly. It is mandatory to recognize and appreciate the significance of these aging processes in planning the perioperative management of the physiologically aged patient.
This review will consider the overall risks associated with the elderly patient, the significant physiologic changes accompanying the aging process, known pharmacokinetic, pharmacodynamic variations and, lastly, some specific anesthetic considerations.
